The Community Care Social Worker/Case Manager works Monday through Friday with no call and no weekends. The role involves working in a multi-disciplinary approach to plan, coordinate, monitor, and supervise the provision of services to consumers enrolled in the Home and Community Based Services Program. The candidate must demonstrate the ability to handle pressure, meet deadlines, be accurate, and manage constantly changing situations, including dealing with a variety of people, stressful situations, and conflict. The job duties include performing all assigned tasks and adhering to all licensing board or agency requirements, regulations, and procedures, as well as complying with Benefis Health System’s policies and procedures. Education and experience requirements include a Bachelor’s degree in social work or related fields, three years of social work experience in a healthcare setting preferred, knowledge of case management methods, diagnostic and crisis intervention skills, issues and needs of long-term care consumers, budget management, and understanding of human behavior, disabilities, and the aging process.
