Position background ? What does this position accomplish for the business? The position will assist with reduction in overall admissions, readmissions and ER visits. The position will assist with acute calls and provide clinical support for acute visits on site. (Triage role)

What are the top 5-10 responsibilities for this position (please be detailed as to what the candidate is expected to do or complete on a daily basis)? Clinical Triage Support – in clinic setting and telephonic support.
? Maintains an active nursing appointment schedule for assisting the Provider in the monitoring of members’ health care needs.
? Assists the Care Manager and Provider in the follow-up management of post-hospitalized or post-emergency room visits.
? Comprehensively screens patients for care opportunities and collaborates with treating physician and/or Registered Nurse to develop care plans that meet patients’ needs and result in appropriate quality care.
? Provides condition specific patient education and anticipatory guidance, as well as care coordination, under the guidance of the Provider and/or Registered Nurse.
? Maintains clinical documentation within the medical records assuring compliance within all guidelines.
? Provides health teaching.
? Comprehensively screens patients for care opportunities and collaborates with treating physician to identify referrals to care management.
What does the ideal candidate background look like (ex: healthcare specific background specific industry experience etc.)? Healthcare professional experience in a hospital setting, urgent care, or primary care. (Preference Medicare Advantaged Care experience)

o Ability to interact and verbally communicate effectively with patients and all levels of personnel in a professional, courteous and effective manner with excellent customer service skills.
o Ability to work in a fast-paced environment. Ability to multi-task; must possess strong organizational and time management skills.
o Excellent interpersonal, verbal and written communication skills.
o Ability to work independently, with minimal supervision.
o Proficient in MS Office and Excel.
o Working knowledge of medical terminology.
o Knowledge of medical equipment and maintenance. Ability to learn how to maintain medical equipment.
o Knowledge of HIPAA regulations.
o Ability to complete a color blindness test.
